Release of Liability : I release the Connected Community, This Generation Connect and The Connect Center, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, losses, or damages arising out of or in any way connected with my participation in any Sunday Funday activities offered by the Connected Community. This includes, but is not limited to, any injuries, illness, death, or property damage that I may suffer.

Assumption of Risk: I understand that participation in any Sunday Funday activities offered by the Connected Community may involve inherent risks, including but not limited to physical exertion, exposure to the elements, and the potential for accidents. I voluntarily assume all risks associated with such participation.

I understand this waiver applies to all names documented within this form including children.
